Hope Care Ride is committed to providing high quality transportation services to/from Park Avenue Senior Care as well as to/from medical appointments (Monday-Friday).

 

Hope Care Ride’s professional drivers will provide you with safe, dependable and courteous service to your destination.  Our drivers are fluent in Chinese and English and will work with you to accommodate your needs.

 

Eligibility/Cost

If you have Medicaid, then you are eligible for transportation to/from Park Avenue Senior Care and to/from your medical appointments.  With Medicaid, there is no cost to you.

 

How do you arrange for transportation?

 

If you are a member at Park Avenue Senior Care, the Nurse Case Managers will arrange transportation to/from the center as well as to all medical appointments for you.  If you are not a member of Park Avenue Senior Care, and would like a tour, please contact us at:
